29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. (a). Plainer area: On or about December 12, 2012 and at times prior to, the employer exposed his employees to electrical hazards in that employees were allowed to operate a 480 V Trimmer Control Center panel that did not have a cover over the exposed 480 V wiring. (b). Stacker area: On or about December 12, 2012 and at times prior to, the employer exposed his employees to electrical hazards in that employees were allowed to operate a 480 V Trimmer Control Center panel that did not have a cover over the exposed 480 V wiring.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. (a) Throughout the facility; On or about December 12, 2012 and at times prior to, the employer exposed his employees to a fire hazard in that employees were allowed to work in areas were not clean and orderly.
