29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about December 10, 2012, an employee removing shingles from a 6:12 pitched bi-level residential roof was not protected from a fall hazard of 12 feet from the eave to the ground below.    (b)  On or about December 13, 2012, two employees installing shingles on the 6:12 and 5:12 pitched sections of the bi-level residential roof were not protected from falls of 12 feet and 22 feet from the eaves to the ground below.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) On or about December 10, 2012, the extension ladder used by employees to gain access to the roof was not positionsed the required minimum 3 feet above the eave or landing of the roof.
