29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) On the south exterior elevation of building #4 located at 1338-1350 Piazz Delle Pallotole in Boynton Beach, Florida: An employees performing painting operations from the basket of aerial lifts approximately 20 ft. above a lower level was not wearing a safety harnesses with a lanyard attached to the boom or basket of that lift, on or about December 13, 2012. b) On the south exterior elevation of building #4 located at 1338-1350 Piazz Delle Pallotole in Boynton Beach, Florida: An employees performing painting operations from the basket of aerial lifts approximately 12 ft. 6 inches above a lower level was not wearing a safety harnesses with a lanyard attached to the boom or basket of that lift, on or about December 13, 2012.

PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
