29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     JOBSITE:      1. The employee, performing framing of the widow's walk were exposed to the hazard of falling approximately 30 feet to the ground.    2. The employee, performing framing of the west roof of the residential house was exposed to the hazard of falling approximately 22 feet to the ground.    3. The employees, performing framing at the open-sided edge at the southside at the 2nd level of the residential house were exposed to the hazard of falling approximately 18 feet to the ground.    4. The employees, performing framing in and around the wall openings at the 2nd level of a residential house were exposed to the hazard of falling approximately 16 feet to the ground.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Note:  29 CFR 1926.21(b)(2) may be cited as an additional violation to support this requirement.    JOBSITE:  The employees, using stairways to have access to upper levels of a residential house were not provided with information and training to recognize hazards and minimize hazards related to stairways.

JOBSITE:  The stairway from the main level up to the 2nd level was not equipped with a guardrail system.
