29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Sea Island Bank Financial Center - On or about December 13, 2012, employees working on a three tier tubular welded frame scaffold were exposed to fall hazards of up to approximately 12 feet, 9 inches onto earthen ground because employees working on the second level were not wearing personal fall arrest systems and the guardrails on the scaffolding did not conform to the requirements of paragraph (g)(4) in that there was an opening between the outside platform and the guardrail systems.        b) Sea Island Bank Financial Center - On or about December 13, 2012, employees working on a three tier tubular welded frame scaffold were exposed to fall hazards of up to approximately 12 feet, 9 inches onto earthen ground because the ends of the platforms were not completely guarded while employees were working on the second level of the scaffold.
