29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall: Northeast corner of the facility located at 1215 Tibbals St., Holdrege, NE - The employer is failing to protect employees from fall hazards by allowing them to carry tools and materials from the ground to the roof of this building using an extension ladder. An employee was injured when he fell 11 feet striking the hard dirt ground below. The employee was carrying a Hilti powder actuated gun while climbing the ladder, lost his balance and fell.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: Northeast corner of the facility located at 1215 Tibbals St., Holdrege, NE - Employees had not been adequately trained on the proper use of portable ladders. Serious hazards involving the usage of the ladder on site were identified during the inspection. An employee was injured when he fell 11 feet striking the hard dirt ground below. The employee was carrying a Hilti powder actuated gun while climbing the ladder, lost his balance and fell.
