29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder: (a)Primo's 1 Construction @ 5203 Horizon Drive, Windsor, CO 80550: On and before December 13th, 2012, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. Employees were using a heavy duty portable step-ladder for access to the exterior top plate. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided. (a)Primo's 1 Construction @ 5203 Horizon Dr., Windsor, CO 80550: On and before December 13th, 2012, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in framing activities while working at the unprotected edges of the second floor top plate on a single-family home. An employee was working on the exterior top plate and protected from falls in excess of 6 feet. The condition exposed employees to a fall hazard of approximately 10 feet. Primo's 1 Construction was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 313720187, Citation Number 1, Item 4a issued on July 9th, 2010, and became Final Order on July 21st, 2010, with respect to a workplace located at 10457 Desert Bloom Way, Colorado Springs, CO 80925.
