29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:        Location: Jobsite, 573 Park Avenue    a) Employees removing and replacing a residential roof were using an extension ladder to access the roof that was not extended three feet above the roof landing, thus exposing employees to falling up to three feet to the ground below on or about 12/06/12.
