29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    In the core making area, an employee was using a mobile sand mixer which had unguarded rotating parts.  The employer did not ensure that machine guarding was used on the sand mixer.

29 CFR 1910.1200(h)(1):     Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    An employee was working with an isocynate-based urethane resin in the core making area.  The employer had not provided the employee with effective information and training prior to their initial job assignment.

29 CFR 1910.95(g)(5)(i):     The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:    In the grinding area, an employee conducting cutting and grinding operations was exposed to continuous noise at 263 % of the permissible daily dose (8 - hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 97.0 dBA during the 386 minute sampling period on January 29th, 2013.  Exposure calculations included a zero increment for the 94 minutes not sampled.  The employee had never received a valid baseline audiogram within 6 months of the intial exposure.
