29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: (a) On or about 12/13/12 - Richard Arlington & 10th Street, Birmingham, AL, safe access was not provided for an employee working from mobile scaffolding exposing him to falls of 8 feet, 4 inches to the ground below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about 12/13/12 - Richard Arlington & 10th Street, Birmingham, AL, an employee working from mobile scaffolding had his 6-foot lanyard attached to the anchor point on the platform exposing him to falls of 8 feet, 4 inches to the ground below.
