29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Press brake area. The employers lockout procedures did not address all forms of energy of the press brakes. There was no mention of what procedures were to be followed to relieve any and all residual or stored hydraulic energy to render the equipment safe before any servicing of the equipment is begun.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Press brake area. A maintenance mechanic was observed conducting repairs and adjustments to a press brake during the walk around inspection. The mechanic, who had not locked out the equipment, was exposed to amputation hazards as he was working within the point of operation of the press brake.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Press brake area. The operators of four press brakes were not protected, at the point of operation, from amputation hazards by guarding, distance, or the use of hand tools.    b) The rear of the press brakes was not guarded to prevent accidental exposure of operators or passersby.  to ingoing nip points

The operator of a 3 ton overhead bridge crane had not been designated by the employer as qualified to perform the specific duties associated with crane operations.
