29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    a) Jobsite- 202-216 Shawmont Ave., Philadelphia, PA - Employees were exposed to injuries, in that they were installing trusses on a residence,  at approximately 30 feet above ground level, without any means of fall protection. Observed on or about 12/17/12.    Note: No abatement certification or documentation required.

29 CFR 1926.503(a)(1):     The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a) Jobsite-202-216 Shawmont Ave., Philadelphia, PA- The employer did not provide fall protection training to his employees who were installing trusses on a three story residence at approximately  30 feet above ground level. Observed on or about 12/17/12.    Note: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)      a) Jobsite- 202-216 Shawmont, Philadelphia, PA-Employees were accessing a window opening by climbing a ladder which did not extend at least 3 feet above the upper landing surface, exposing employees to injuries. Observed on or about 12/17/12.    Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
