29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. In the following instances electrical equipment was not used in accordance with instructions included in the listing and labeling: a) Duck Brook Pump Station - The 15 amp relocatable power tap was connected to an extension cord, temporary construction outlet, refrigerator, and microwave. b) Duck Brook Pump Station - The outlet box under the computer station was not securely fastened in place. c) Duck Brook Pump Station - The nonmetallic sheathed cable was not secured at intervals of 4.5 feet.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. Duck Brook Pump Station - The basement floor hole was not protected with a cover. T-Buck Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection number 112523485, Citation 1 Item 2, and was affirmed as a final order on 6/12/2008, with respect to a workplace located at Auburn, Maine.
