29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 12/17/12, on construction site at the Sunoco Food Mart, 211 Niagara St., Buffalo, NY:  An employee of a subcontractor was on the narrow aluminum "pik" of a ladder jack scaffold erected on the south side of the building. The employee was installing soffits, and was exposed to a fall of approx. 15 ft. to the ground. The employee was wearing a full body harness, but was not tied off to an anchor point, and no other fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED
