29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Employees bypassed guards to reach in and remove paper from the Hantscho 4-Color Web Press while following inadequate Lockout/Tagout instructions which required only that the machine be shut off. When employees were placing body parts into the danger zone the machine should have been either locked out or the employees protected by an alternate protection method. The documentation should include written verification of abatement, applicable measurement or monitoring results, and photographs or videos which you believe will be helpful.
