29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) On or about December 18, 2012, the employer did not assure that the emergency exit door located in the warehouse of the weatherization building was able to be opened at all times. The door was locked with a pad lock exposing employees to hazards in the instance the door is needed to be used for evacuation purposes.

The bundles of insulation were stacked in a manner that exposed employees to struck-by hazards when they are being retrieved for use.
