29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied.    Worksite: The supported scaffold platform/metal basket, from that the employees were working at a height approximately 13 feet 9 inches above the ground was not positively secured to the forks of the JLG All-Terrain material handler.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks, "When leaving a powered industrial truck unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shut off, brakes set, key or connector plug removed":    Worksite: The JLG All-Terrain material handler/powered industrial truck (PIT) was left unattended while employees were working from the basket that was elevated at a height approximately 13 feet 9 inches from the ground by the retractable arm/boom of the PIT.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest sytems or a monitoring system.     Worksite: The employee who was walking/working near the edge of the flat roof at a height of approximately 16 feet above the ground was not protected from fall hazards by the use guardrails, safety nets, a personal fall arrest system or a monitoring system.    Xtreme Drywall & Accoustics, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(14) which was contained in OSHA inspection number 314981481, citation number 1, item number 2, issued on 1/30/2012 with respect to a workplace located at 3026 East Main St., Cortland Manor, NY and became a final order on 2/21/2012.
