On or about December 12, 2012 - at the above addressed jobsite, employee and subcontractor employees performing construction activities were exposed to electrical hazards due to the temporary lights having exposed wires and being suspended by their live cords.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. a. On or about December 12, 2012 - at the above addressed jobsite, an employee performing carpentry work from a form scaffold, was exposed to falls to the outside of the building approximately 150 feet to the ground, and approximately 15 feet to the exterior deck created by the lack of fall protection.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. On or about December 12, 2012 - at the above addressed jobsite, employees conducting construction operations on the top deck of the site were expsoed to falls when the employee accessed the exterior of the form by climbing through an opening. The change in elevation was approximately five feet two inches(5'2"), thereby exposing the employee to fall hazards.
