29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;    a. On or about December 12, 2012 - at the above addressed jobsite, employees conducting siding operations from a ladder jack scaffold approximately nineteen (19) feet above the ground were not protected from falls due by a personal fall arrest system thereby exposing the employees to fall hazards.

29 CFR 1926.454(a)(1):   The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include, but is not limited to the following:  The nature of any electrical hazards, fall hazards and falling object hazards in the work area;    a. On or about December 12, 2012 - at the above addressed jobsite, employees  conducting siding operations from a ladder jack scaffold approximately nineteen (19) feet above the ground were not trained in scaffoldiing requirements, thereby exposing the employees to fall hazads.
