29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about December 18, 2012, employees installing plywood roof decking onto a 5:12 pitched roof were not protected by personal fall arrest or any other conventional or alternative fall protection system to safeguard them from a fall hazard of 18 feet.
