29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees: On or about December 18, 2012, at 12764 Raul Mendiola Ln., the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees: 1. A 100-foot yellow extension cord used to power the RYOBI Circular Saw had extensive damage to the outer insulation. 2. A 100-foot orange extension cord used to power the Makita Circular Saw had extensive damage to the outer insulation. 3. The power cord on the RYOBI, 7 -inches Circular Saw, Model No. CS1147-81749, was cut through exposing bare conductors.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose. On or about December 18, 2012, at 12764 Raul Mendiola Ln., a hand-made electrical handybox with three duplex receptacles (15A, 125VAC, 2-pole, 3-wire) made for indoors was used outdoors. The handy box with no cover exposed the electrical bare conductors.
