29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    On or about December 19, 2012, employees required to work (i.e. skim) at the molten zinc galvanizing kettle when parts are being lowered, or submerged, were exposed to the hazard of burns.  Protective clothing, such as flame-retardant coveralls, was not provided to and used by employees to protect against injury.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use:    On or about December 19, 2012, outside of the facility, employees required to load a storage tank with sulfuric acid (70-100%) were exposed to the hazard of chemical burns.  Suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  An emergency eyewash was not available and the available emergency shower unit delivered frigid unheated brown-colored water. 29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:    On or about December 19, 2012, in the facility, employees required to peform steel galvanizing operations were exposed to contact with hazardous chemicals, including, but not limited to, sulfuric acid and zinc chloride.  Employees had not been provided effective information and training on the hazards of the chemicals in their work area, including:    Information:    (i) The requirements of this section;  (ii) Any operations in their work area where hazardous chemicals are present;  (iii) The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and material safety data sheets required by this section.    Training:    (i) Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released);  (ii) The physical and health hazards of the chemicals in the work area;  (iii) The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used;  (iv) The details of the hazard communication program developed by the employer, including an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information.
