29 CFR 1910.23(a)(5):  Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction.  While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:  Laundry Building:  On or about December 17, 2012, and at times prior thereto, employees were exposed to the hazards of slip, trip, and falls on an uncovered drainage pit when accessing the area to clean behind the machines and/or changing the laundry chemical containers.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Laundry Building:  On or about December 17, 2012, and at times prior thereto, the employer did not ensure the eyewash station was easily accessible for employees working with corrosive liquids such as, but not limited to, ECOLAB Laundry Tri-Star L-2000 XP Detergent, Laundri Destainer, and Tri-Star Neutralizer.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a)  Laundry Building:  On or about December 17, 2013 and at times prior thereto, a portable fire extinguisher was obstructed by boxes and cleaning materials.  In the event of a fire, the portable fire extinguisher may not be readily accessible.    b)  Laundry Building:  On or about December 17, 2013 and at times prior thereto, a portable fire extinguisher was not fully visible and its location was not identified.  In the event of a fire, the portable fire extinguisher may not be easily located.

29 CFR 1910.1030(f)(2)(i):  Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure:   On or about December 17, 2012, and at times prior thereto, employees were asked to sign declination forms upon initial assignment.  The employer did not make the Hepatitis B vaccination series available to housekeepers and laundry aides who were occupationally exposed to blood or other potentially infectious materials until after a couple months of employment with the company.  Healthcare Services Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1030(f)(2)(i), which was contained in OSHA inspection number 311577423, citation number 1, item number 1 and was affirmed as a final order on 11/10/2008 with respect to a workplace located at 8211 Ustick Road, Boise, ID 83704.
