29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. (a) On or about December 19, 2012, at bldg 14 of lot 127, the tubular supported scaffold platform where employees were working from a height of 14 feet was installed with open spaces of 10 inches exposing the employees to a fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) On or about December 19, 2012, employees on a supported tubular scaffold were exposed to a fall hazard of 14 feet where the scaffold platform did not have a complete guardrail system or any other means of fall protection.
