29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about 12/19/12 an extension cord was found that did not have a ground prong. The cord was being used by a laborer using a Makita saw, serial number 1005577 exposing worker to an electrical hazard at the construction site.
