29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a)  Brunswick Home Improvement, LLC on or about December 12, 2012, at the address of 4605 Memorial Shoreway Marblehead, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees were exposed to a 25 foot fall to the ground below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  Brunswick Home Improvement, LLC on or about December 12, 2012, at the address of 4605 Memorial Shoreway Marblehead, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems, guardrails or alternative measures while performing roofing operations on a residential type structure.
