On or about Thursday December 20, 2012 in the warehouse upper storage area of the facility; The area over the office and restroom was used by the employees and used as a storage area for material such as but not limited to books, boxes of papers, and staging material was not load approved and marked for the floor rating.

29 CFR 1910.178(q)(6): Industrial trucks was altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, and altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks was done without being approved by the truck manufacturer. On or about Thursday December 20, 2012 in the warehouse area of the facility; An employee operating a powered industrial truck in a warehouse used a non-manufacturer approved work platform to elevate employee at heights of 11 feet to retrieve material off the storage racks.
