29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    The employer did not develop and document a personal protective equipment hazard assessment for the tasks performed by employees performing vehicle maintenance in the shop area, and the written certification did not include the name of the person who certified that the evaluation had been performed, and the date that the hazard assessment was completed.

29 CFR 1910.1200(h)(1):     Employees were not provided information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:    Employees were exposed to physical and chemical hazards when charging forklift batteries and cleaning parts and were not provided information and training on hazardous chemicals in their work area.    Employees shall be informed of:    1. The requirement of this section;  2. Any operation in their work area where hazardous chemicals are present;  3. The location and availability of the written hazardous communication program.    Employee training shall include at least:     1. Methods and observations to detect the presence/release of hazardous chemicals;  2. The physical and health hazards of the chemicals in the work area;  3. The measures employees can take to protect themselves;  4. The details of the hazard communication program developed by the employer.
