Observed on or about 12/05/12.

The employees did not have the knowledge to recognize the hazards of falling and the procedures to protect themselves from such hazards. Observed on or about 12/05/12.

The ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access to the roof. Observed on or about 12/05/12.
