29 CFR 1910.22(c):     Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.:    On or about January 16, 2013, at the facility located at 201 North 1st Street in West Helena, Arkansas, Press Department, the approximately 12" wide x 10" deep pit to the glue spreader machines was not covered.  This condition exposed employees to falls from the same elevation.

29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    On or about January 16, 2013, and times prior to, at the facility located at 201 North 1st Street in West Helena, Arkansas, Press Department, standard railing was not provided on:    a.  The elevatable platforms on the front and back of the Hot Press machine.  Employees were exposed to falls from elevations.    b.  The approximately 8 x 20 ft. platform assessed by employee to perform glue batch mixing activities.  Employee was exposed to an approximately 5 ft. fall from elevation.

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about January 16, 2013, and times prior to, at the facility located at 201 North 1st Street in West Helena, Arkansas, the employer did not include machine specific isolation procedures for the benefit of employees required to service and/or perform maintenance on equipment such as but not limited to the Feeder, Hot Press, Molder, Hydromat and Hand Scraper machines.  Failure to do so exposed employees to caught-in and/or contact with rotating parts if the machine was unexpectedly energized.
