This violation was observed on or about December 17, 2012, on the north side of a home being constructed when an employee was exposed to a fall hazard while on the working level of a pump jack scaffold without a guardrail system in place.     b. This violation was observed on or about December 17, 2012, on the west side of a home being constructed when an employee was exposed to a fall hazard while on the working level of a pump jack scaffold without a guardrail system in place.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds are constructed an maintained in accordance with 29 CFR 1926.451 or 29 CFR 1926.452.

Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.
