On or about 12/20/2012, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2012: Throughout the workplace: On 12/20/2012, the employer did not fill out an OSHA Form 301 or equivalent for the work related injuries that occurred on the following dates during the 2011 calendar year: a. 08/23/2011--Welding Area--The eye of an employee was struck by foreign matter while the employee was engaged in welding activities. b. 09/16/2011--West end of Scale--The back of an employee was injured while engaged in worksite trash shoveling activities. c. 11/08/2011--Shredder Site--The feet of the employee were struck by flying metal while engaged in Shredder support operations. d. 11/10/2011--Non Ferrous Area--Employee engaged in non ferrous metal support activities tripped upon debris stored upon the ground. e. 11/16/2011--Shredder Site--The feet of the employee were struck by flying metal while engaged in Shredder cleaning operations.
