29 CFR 1926.501(b)(7)(i): Each employee at the edge of an excavation 6 feet (1.8 m) or more in depth shall be protected from falling by guardrail systems, fences, or barricades when the excavations are not readily seen because of plant growth or other visual barrier; Jobsite: Employees were exposed to falls greater then six feet next to an open excavation with no guardrails to prevent employees from slips, trips or falls.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired. Jobsite: Employees were using an extension laddert hat was taken apart due to one piece being broken. The piece that was on site was the extension part which has no foot pegs and employees stated they did use it at one time.
