29 CFR 1926.501(b)(1):  "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a.  On or about December 20, 2012, on the north side of the 1st, 2nd, 3rd, and 4th floors, employees were using stairs near an unprotected edge and were not protected from a 32 feet fall hazard, by the use of a fall protection system.

29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a.  On or about December 20, 2012, on the north side of the 3rd floor, employees were working near floor holes that were not covered, exposing themselves to a trip hazard.

On or about December 20, 2012, on the north side, the stairs leading from the 1st to the 2nd floor did not have a stair rail along an unprotected side.
