29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a.  On or about December 20, 2012, on the north side of the 3rd floor, employees working near the edge were not protected from a 30 feet fall hazard by the use of a fall protection system.    b.  On or about December 20, 2012, on the south side of the 1st and 2nd floor, employees working near the edge were not protected from a 30 feet fall hazard by the use of a fall protection system.

On or about December 20, 2012, on the 3rd floor, employees were working near floor holes that were not covered, exposing themselves to a trip hazard.

29 CFR 1926.1052(c)(1):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:    a:  On or about December 20, 2012, on the east side of the building, the stairs leading to the 2nd floor did not have a stair rail along an unprotected side.     b.  On or about December 20, 2012, on the west side of the building, the stairs leading to the 2nd floor did not have a stair rail along an unprotected side.
