29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The Delta radial arm saw in the Shop Area used to cut foam was not guarded. The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) The propane heater in the Paint Room did not have adequate strain relief for the electrical wiring. The electrical wiring to the secondary gas valve was not connected exposing interior wiring. (b) There was not adequate strain relief for the Delta Radial Arm Saw in Global Therm, as the wiring had pulled back from where it was connected to the motor, exposing interior wiring.

(b) An accident occurred on or about September 19, 2012 when an employee received an injury which was recordable and was not entered on the OSHA 300 log within seven calendar days. (c) An accident occurred on or about September 22, 2012 when an employee received an injury which was recordable and was not entered on the OSHA 300 log within seven calendar days. (d) An accident occurred on or about October 10, 2012 when and employee received an injury which was recordable and was not entered on the OSHA 300 log within seven calendar days.
