Band Oven - employees were exposed to a fall of at least 6 feet while changing filters on top of the Band Oven.    IN THE ALTERNATIVE:    29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a.) Band Oven - employees were exposed to a fall of at least 6 feet while changing filters on top of the Band Oven.

1910.133(a)(1) The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids:    a.) Chemical Mixing Area - Employees were exposed to Sani T 10 Plus sanitizer while dispensing from a 55 gallon drum into a 5 gallon bucket.  The sanitizer contains quartenary ammonium chloride which is corrosive.  Employees were not required to use chemical splash goggles, face shields or other types of eye and face protection.

29 CFR 1910.132(d)(2): The employer did not verify through a written certification that the required workplace hazard assessment has been performed  that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.      a.) iBake Foods, LLC -  Employees were periodically exposed to various hazards including, but not limited to, chemical exposure hazards to the eyes, face, hands and arms.  Adequate personal protective equipment was not available or required to be used by employees.  The facility did not maintain the documentation for the hazard assessment conducted in the workplace.

Chemical Mixing Area - Employees were exposed to Sani T 10 Plus sanitizer while dispensing from a 55 gallon drum into a 5 gallon bucket.  The sanitizer contains quartenary ammonium chloride which is corrosive.  Employees were provided standard, store-bought dishwashing-type gloves.

Chemical Mixing Area - Employees dispense Sani T 10 Plus sanitizer, which is corrosive, out of a 55 gallon drum via a manual pump into a 5 gallon bucket.  No eyewash facilities were provided in the area.
