29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    (a)Smiley, Inc. @ 2955 4th Street, Boulder, CO 80302: On and before February 14, 2011, the exposing employer did not ensure that each employee engaged in the installation of flashing on the roof of a single-family home, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The employer did not provide training on the simple saver fall protection system involving it's limitations and use. The condition exposed employees to the hazard of falling from an elevation of approximately 15'.
