29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a)Mobile Maintenance & Construction, Inc. @ 2146 Hecla Dr., Louisville, CO 80027: On and before December 19th, 2012, the exposing employer did not ensure that employees were protected from injury, in that employees conducting tarping operations on the roof of a multi-family home were not utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a)Mobile Maintenance & Construction, Inc. @ 2146 Hecla Dr., Louisville, CO 80027: On and before December 19th, 2012, the exposing employer did not ensure that each employee engaged in tarping operations on a new multi-family residence, had been trained to recognize the hazards associated with working at elevations over 6'. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevation of approximately 20 feet.
