29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Jobsite located at 11646 HWY 319 N, Thomasville GA - On or about, and at times prior to December 19, 2012, employees were allowed to operate a GENIE forklift, model GTH-844, without forklift certification and training, thus resulting in employees being exposed to ejection, struck-by and crush-by hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. a) Jobsite located at 11646 HWY 319 N, Thomasville GA - On or about, and at times prior to, December 17, 2012, the employer used a Genie GTH 844, telescopic forklift to support and elevate personnel with an unapproved work platform, thus exposing employees to ejection, struck-by, crush-by, and fall hazards up to about 18 feet. The forklift manufacture does not allow this model to have attached any form of personnel platform or allow personnel to ride on attachment. ABATEMENT CERTIFICATION IS REQUIRED
