29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Jobsite at corner of Grant and North Third Street, Duquesne, Pennsylvania 15110: Observed on or about December 20, 2012, an employee engaged in demolition activities while working from the basket of an aerial lift was not wearing a protective helmet. The employee was exposed a head injury hazard from the loose debris located above the employee.

1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Jobsite at corner of Grant and North Third Street, Duquesne, Pennsylvania 15110: Observed on or about December 20, 2012, an employee engaged in demolition activities while working from the basket of an aerial lift equipped with an articulating boom was not wearing a harness with a lanyard attached to the boom or basket. The employee was exposed to a fall hazard of approximately 20 feet to the ground below.
