29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Jobsite at corner of Grant and North Third Street, Duquesne, Pennsylvania 15110:  Observed on or about December 20, 2012, an employee engaged in demolition activities while working from the basket of an aerial lift was not wearing a protective helmet.  The employee was exposed a head injury hazard from the loose debris located above the employee.

1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Jobsite at corner of Grant and North Third Street, Duquesne, Pennsylvania 15110:  Observed on or about December 20, 2012, an employee engaged in demolition activities while working from the basket of an aerial lift equipped with an articulating boom was not wearing a harness with a lanyard attached to the boom or basket.  The employee was exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards.    a) Jobsite at corner of Grant and North Third Street, Duquesne, Pennsylvania 15110:  Observed on or about December 20, 2012, the employer did not provide fall protection training to an employee engaged in demolition activities.  The employee was exposed to a fall hazard of approximately 20 feet to the ground below while working from the basket of an articulating boom aerial lift.
