29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Throughout the production area; The employer did not perform a periodic inspection of the energy control procedures on an at least annual basis for equipment including but not limited to; Hurco VMX 64, Hurco BMC 40, Hurco BMC 50, Hurco BHT and the AWEA 460.    The employer, Kobra Molds LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 313176984, citation number 1, item number 2c and was affirmed as a final order on July 08, 2010, with respect to a workplace located at 2801 Enloe St, Hudson, WI 54016.

29 CFR 1910.134(d): Employer did not evaluate respiratory hazard(s) in the workplace, to identify relevant workplace and user factors, and base respirator selections on these factors:  a) At the paint spray booth; The employer did not evaluate the respiratory hazards in the spray booth that were associated with the use of chemicals such as but not limited to; ALV-Aerosol, Acrylic Modified Alkyd Enamal and Acetone. The health risk rating on two of these chemicals is a 3 according to the Health Material Information System on the material safety data sheets.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them:  a) Throughout the production area; The energy isolation procedures for equipment including but not limited to; the CMM, Hurco BMC 40, Hurco BMC 50 and the Grinder, did not clearly and specifically identify the magnitude and location for the electrical disconnect.
