On or about 12/18/2012, employees from Pasha Distribution Services located at 8050 Veterans Memorial Parkway, St. Peters, MO 63376 were using a Delta bench grinder while grinding miscellaneous metal parts.

29 CFR 1910.215(d)(1): Inspection: Immediately before mounting, the wheel was not closely inspected and sounded by the user (ring test) to make sure they have not been damaged in transit, storage, or otherwise. On or about 12/18/2012, employees from Pasha Distribution Services located at 8050 Veterans Memorial Parkway, St. Peters, MO 63376 were using a Delta bench grinder with a grinding wheel installed, which had not been ring tested prior to mounting/use.
