29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 20 Memorial Court, Building 5, Denville, NJ and at the Corporate Offices at 30 Sioux Avenue, Lake Hiawatha, NJ. The employer exposed employees to explosion injuries as employees were not instructed in the recognition and avoidance of unsafe conditions such as, but not limited to digging a trench without estimating the location of underground utility installations. Violation observed on or about 12/06/2012.

29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not determined prior to opening an excavation: a) 20 Memorial Court, Building 5, Denville, NJ. The employer exposed employees to explosion injuries as underground utilities were not located prior to digging resulting in a 3/4 inch gas line being pulled out from its curb stop. Violation observed on or about 12/06/2012.
