29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. Inside of the maintenance shop, there was a Delta drill press being used with no chuck (arbor) guard in place. Employees were exposed to caught in and struck by hazards.

29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving. 1. Inside of the maintenance shop, employees were using a Delta drill press that was not anchored to the concrete floor to prevent movement. Employees were exposed to caught in and struck by hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. 1. Inside of the button assembly area, on November 6, 2012, an employee was able to get her hand under a guard and into the point of operation zone. This button assembly machine was inadequately guarded and the employee should not have been able to get her hand under the guard. This employee received a crushed middle finger and an amputation to the tip of the left index finger. Company immediately installed a better guard on the button assembly machine following the accident.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. Inside of the button assembly area, employees were using a Toledo (weighing) scale that was not grounded. The metal casing (framing) of the equipment was energized. 2. Inside of the cell E area, employees were using a strapping machine (#44) that was not grounded. The metal casing (framing) of the machine was energized. 3. Inside of the cell C area, employees were using a strapping machine (#34) that was not grounded. The metal casing (framing) of the machine was energized. 4. Inside of the cell D area, employees were using a strapping machine (#19) that was not grounded. The metal casing (framing) of the machine was energized. 5. Inside of the maintenance shop, employees were using a Hatachi table saw that was not grounded. The ground pin was missing from the plug on the electrical cord, and the metal casing (framing) of the tool was energized.
