29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about December 21, 2012, employees performing roofing activities on a residential roof with a 16:12 pitch roof were not wearing fall protection and did not have a fall protection system. Employees were exposed to a fall hazard of greater than 10 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about December 21. 2012, employees performing roofing activities on a residential 16:12 pitch roof gained access by means of portable ladders. The ladders did not extend at least 3 feet above the upper landing surface exposing employees to the hazard of falling approximately 10 feet and 12 feet to the ground below.
