29 CFR 1910.146(c)(3): When the employer decided that its employees would not enter permit spaces, the employer did not comply with 29 CFR 1910.146(c)(1), 29 CFR 1910.146(c)(2), 29 CFR 1910.146(c)(6), and 29 CFR 1910.146(c)(8: (a) Vessells, Outside - On or about December 7, 2012 the employer failed to evaluate the workplace and post the signs indicating the vessels containing propylene pellets as a confined space.
