29 CFR 1910.178(q)(7): Industrial trucks were not be examined before being placed in service, and/or were placed in service if the examination showed any condition adversely affecting the safety of the vehicle: On, or about, December 6, 2012, an employee suffered a significant static shock while standing on a Raymond side loader industrial truck, RO-16. The secondary bonding strap was found to be missing/broken off when the vehicle was inspected following the incident. The company had previously installed grounding straps following another static shock incident, but had not upated the daily inspection program to verify that grounding straps/chains were present and in good condition prior to each day's use of the side loaders.
