29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket: (a) 1701 Sunrise Hwy, Westfield South Shore Mall, Macy's (Worksite), east entrance - Employees climbed and stood on the JLG Model 660SJ aerial lift basket, Serial # 094520 0300042590, to increase their working height while they connected the structural steel for the east entrance, on or about December 26, 2012. (b) (Worksite), second floor above east entrance - An employee used the railings of the JLG Model 660SJ aerial lift basket, Serial # 094520 0300042590, to climb to and from the second floor, which was approximately 20 feet above the ground, on or about December 26, 2102. NOTE: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
