29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    a) 803 Kennedy Blvd, Bayonne, NJ.    Subcontractor's employees were not made to utilize personal protective equipment while in the process of framing a roof. Employees were observed using a nail gun without any means of eye/face protection.                                 Violation observed on or about 12/19/2012.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    803 Kennedy Blvd, Bayonne, NJ.    Subcontractor's employees were observed working on roof top at the edge without any means of fall protection. The GC i.e. owner was on site observing his subcontractor's employees working roof top edge 30 feet high without any means of fall protection being utilized, thereby exposing his subcontractors employees to a fall hazard of 30 feet to the ground.                                         Violation observed on or about 12/19/2012.    b) 803 Kennedy Blvd, Bayonne, NJ.    Subcontractor's employees were observed working on roof top at the edge without fall protection being used, thereby exposing his subcontractor's employees to fall hazard of 30 feet to the ground.                                         Violation observed on or about 12/20/2012.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  803 Kennedy Blvd, Bayonne, NJ.    Employees used a portable ladder to access and egress from the roof top that extended approximately 1 foot above the landing, thereby exposing the employees to a fall hazard of 35 feet to the next lower level.                             Violation occurred on or about 12/19/2012.
